About Us

Nestled in the Endless Mountains of Northeastern Pennsylvania, Twin Oaks Personal Care Home offers a beautiful setting, spacious rooms, personal service and home-cooked meals.  Residents quickly become “part of the family” in our beautiful, elegant facility.  Known as the former Granville Elementary School this facility was transformed into a cozy, comfortable place for seniors.  Friendly staff offer a nurturing caring environment yet allow residents to remain independent.  Twin Oaks is truly ”a place to call home”.

Twin Oaks Personal Care Home

Residents at Twin Oaks enjoy a cozy, comfortable private or semi-private room. These rooms come furnished and offer cable for their television. Hook-ups for telephone and computer are also available. Each room is designed to be home-like and safe with an emergency call system.

A spacious dining/activity room is where the meals and special activities normally take place. This room is a place where residents gather for meals as well as visit with other residents and watch television or participate in crafts, exercise, listen to special speakers and attend many other events.


Twin Oaks Personal Care Home strives to offer personal assistance while allowing our residents to continue to maintain an independent life-style.  We offer many services to make this possible with each service geared toward the individual and their personal needs.

  • Assistance with activities of daily living:
    eating, drinking, ambulating, transferring in and out of bed or chair, toileting,
    bladder and bowel management, personal hygiene, securing health care,
    managing health care, self-administering medication and proper turning and
    positioning in a bed or chair

Prior to, or within thirty days of admission, a resident must provide documentation from his/her physician that he/she is eligible for personal care home living.


We do not provide the following services but will assist the residents in acquiring if needed or desired:

Medical services requiring a nurse or physician – Transportation exceeding 4o miles from the home – Handling of resident’s financing – Dry cleaning

Twin Oaks Personal Care Home does not have the capability to serve persons with complex medical and mental health needs, ie:

  • Residents who are considered to be a health risk
  • Residents who require 2 assists on transferring or ambulation

Prior to admittance, the administrator does a pre-admissions assessment as required by state regulations.
